A case study involving male dichorionic-diamniotic twins where hypospadias was noted in the delivery suite. First pass urine samples for toxicology was requested and both twins tested positive for cocaine metabolites in their urine.

Hypospadias is a common congenital anomaly. As the use of cocaine is increasing over time there may be an under recognition of this association and a missed opportunity to correctly identify a cause.
