Good practice in care planning is a UK good practice guide, based on reports from areas that performed well in the 2005 / 2006 Healthcare Commission and NTA Improvement Reviews. From interviews with key informants in these areas, a number of recurring themes were highlighted which they believed were instrumental to their scoring well in the review. These factors influencing good performance in care planning are set out, along with illustrative case studies from particular areas.
